FirstHand Care provides person-centered Medicaid waiver services in Kentucky, organized by waiver and program so participants, families, guardians, and case managers can quickly find the right supports.

SCL

Supports for Community Living (SCL)

Person-centered supports that help individuals live safely, build daily living skills, and take an active part in their communities.

Residential Level I – 3 or Fewer Residents

Person-centered residential supports provided in a community setting for three or fewer individuals. Our goal is to promote independence, safety, daily living skills, community involvement, and achievement of each participant’s individualized goals.

Adult Day Training

Individualized day services focused on developing and maintaining skills, increasing independence, building social connections, and providing meaningful opportunities for community participation.

Adult Day Health Care

Structured daytime services that combine meaningful activities and community participation with health-related supports and supervision based on each participant’s individual needs.

Positive Behavior Supports

Individualized behavioral services designed to identify needs, strengthen positive skills and behaviors, and support greater independence and successful participation at home and in the community.

Functional Analysis / Consultative Clinical & Therapeutic Services

Assessment and consultation used to better understand factors affecting an individual’s behavior and to develop appropriate strategies and recommendations for the participant and their support team.

Positive Behavior Support Plans

Development of individualized positive behavior support strategies based on the participant’s assessed needs, goals, environment, and person-centered plan.

HCB

Home and Community Based (HCB) Services

Individualized assistance that helps participants remain safe and as independent as possible in their own homes and communities.

Attendant Care

Individualized assistance that helps participants safely perform daily activities and remain as independent as possible in their homes and communities. Services are based on each participant’s assessed needs and person-centered plan.

Adult Day Health Care

Daytime services providing health-related supports, supervision, activities, socialization, skill development, and opportunities for community participation based on individual needs.

HCB · Case Management

Helping Individuals Navigate Services & Achieve Their Goals

As part of our Home and Community Based (HCB) services, FirstHand Care provides person-centered case management to help individuals and families understand, coordinate, and access the supports and resources needed to achieve greater independence and quality of life.

Our case management services may include:

  • Development and coordination of person-centered services
  • Assistance identifying appropriate services and community resources
  • Coordination with participants, families, guardians, providers, and other members of the support team
  • Monitoring services to help ensure they continue to meet the participant’s needs and goals
  • Assistance navigating waiver services and available resources
  • Supporting participant choice, independence, health, safety, and community involvement
  • Ongoing communication and follow-up with the participant and their support team
